Digitally Delivered Peer Support

As the COVID-19 pandemic continues, and social distancing becomes the new normal, we have seen rapid expansion of digitally delivered peer and family support. In this post, we explore emerging issues and considerations for using technology to reach others.

The use of technology to offer peer support is a practice that has been around for quite a while. Think about telephonic peer support via peer-operated warmlines. A from December 4, 2019 said this about warmlines:

“Unlike a hotline for those in immediate crisis, warmlines provide early intervention with emotional support that can prevent a crisis.”

Magellan has long been a supporter of peer-operated warmlines and has helped launch warmlines in multiple states.

As opportunities for in-person, face-to-face support have decreased, many peer-run organizations, along with traditional mental health providers, have shifted delivery of services to telehealth platforms. Yet, during this unprecedented COVID-19 public health crisis, we see increased demand for support from folks living with anxiety and depression. As more families are impacted by the pandemic, access to peer support is more critical than ever.

Peers have been quick to step up with a national grassroots-driven expansion of support via video conferencing platforms like Zoom. With this rapid expansion, however, we see wide variability in practices. Skills learned by providing in-person peer support do not necessarily translate onto a digital delivery platform. Individuals who are used to seeing a peer supporter in person may have a different experience when it’s offered through technology. In some cases, apps that purport to offer peer support are actually “peer-bots,” with real-time interactions being driven by algorithms and predictive analytics.

Fortunately, solutions to address the nuances of digital peer support have been developed. One of the most beneficial is training in digital peer support. Magellan partnered with Dr. Karen Fortuna to provide specialized training to our team members who provide peer and family support. Many other organizations and agencies have completed this training as well. It’s an important and necessary step to take as we navigate this new normal.

The Lived Experience Factor in Fighting the Opioid Epidemic

“Strung out. Looking for the next fix. A full time job that doesn’t pay, but just takes.”

That’s how one of Magellan’s Recovery Support Navigators defined her addiction to opiates. She has since overcome the stranglehold of addiction and is now in long term recovery.  Her experiences, and those of her fellow Recovery Support Navigators, provide the credibility needed to influence their peers who are still struggling with addiction and mental illness that recovery is possible and that desperation does not have to define a life.

An estimated 20.2 million Americans live with a substance use disorder[1]. That’s the population of my home state of Florida. Of that, 2.1 million Americans live with an opioid disorder. Similar to serious mental illnesses, substance use disorders can be difficult to address. Those with the lived experience of walking the path from hopelessness to wellness are experts at engaging individuals with the services and supports that foster the recovery process. Stories of changed lives are not the only proof.

A January 2018 study of our internal recovery support navigation program at Magellan Health found statistically significant improvements for individuals living with substance use and mental health disorders. A key component of the Magellan’s internal peer support program is meeting people where they are. The first engagement with members often happens in an emergency room, inpatient setting, or on the streets. Members then choose where future meet-ups happen. Often, that choice is in the comfort of their own home.  If they are experiencing homelessness, the member chooses a safe place.

The study compared utilization of services six months prior to receiving peer support services and six months following the completion of peer support services. Seventy-five percent of the members in the study had a co-occurring mental health and substance use disorder and 50 percent had a co-occurring substance use disorder, mood disorder and psychotic disorder. The study found that members in Magellan’s internal peer support program cost the health plan significantly less in the six months following completion of the peer support program than members who only had care management or who did not receive any intervention from the health plan. The study also found that six months after completing peer services, peer support decreased emergency room and inpatient hospitalizations.

The role modeling by a peer specialist of a life lived well in recovery is evidence for the individual struggling that there can be long term sustainment of recovery. In addition, the data from this study replicates prior findings that peer support has a lasting impact on the lives of those living with substance use and mental health disorders.

MY LIFE: Youth-Inspired and Inspiring

Mike* was dancing wildly to hip hop music before running up to me and confidently stating, “I’m a leader.” He told me about how he ran to be a class officer and lost but how he was not going to give up. He talked about his search for a life-long mentor and how he wanted to make the world a better place. Not the type of random conversation you would expect at an outdoor community event, but MY LIFE youth are not average.

Since 2007, Magellan Youth Leaders Inspiring Future Empowerment, or MY LIFE, has proactively sought to change the trajectory of young lives engaged in the foster care and juvenile justice systems, as well as youth with behavioral challenges. Led by Greg Dicharry, Magellan’s national director for youth empowerment, monthly MY LIFE groups and annual MY FEST events have provided thousands of youth an opportunity to connect to their community and hear stories of overcoming hardships and reaching for their full potential. In Florida alone, groups in Tallahassee, Orlando and West Palm Beach enable over 100 youth ages 13 to 23 to meet monthly in a safe environment where fun and inspiration are plentiful.

Getting to spend time with youth involved in MY LIFE underscores the impact. One young man who has been attending a Florida MY LIFE group for three years described how the motivational speaker’s life lessons inspired him to focus on his future instead of focusing on his past. He told me about the hard times he was going through now and how the stories of the speakers going through tragic times provided him with the feeling that “it’s not too late for me.” It was a tough thought to imagine that at only 16 years old, this young man felt like the cards had already been stacked against him. MY LIFE provides a consistent message that he and his peers have the power and support to create a future different from their current reality.

And then there was Mike with his boundless energy. He may never lose that energy, which could either lead him down a path that is damaging to himself and others, or one that honors his strengths and abilities as he contributes to society. MY LIFE has helped him set upon the latter path. His future can now be one where his energy is directed into developing his leadership skills, pursuing higher education and a meaningful career, despite the challenges he might face at home or at school.

MY LIFE is just one way Magellan is supporting young leaders to help make our communities stronger.

Strength in the Storm: Thinking about Suicide in the Face of Natural Disasters

Contemplating life and death is probably common when in the midst of a natural disaster. As the eye of Irma passed over me on September 10, I was thinking about suicide and suicide prevention. September is Suicide Prevention Awareness Month, and Magellan Complete Care, our health plan for individuals living with serious mental illness in Florida, had been busy preparing for summits to draw attention to and dialogue around suicide prevention to local communities. Two days before Irma hit, I was busy canceling vendors for the summits. The day before Irma, I was dealing with a surprise, last-minute mandatory evacuation order while trying to locate a friend who was experiencing homelessness and depression, and who had expressed a desire to let the storm take his life instead of seeking shelter.

As the winds howled, windows rattled, and rain pounded against the home I had evacuated to, I sat on a closet floor thinking about how much I wanted to live and how that desire to live had not always been there. There were years where, like my friend, I was lost in my own storm. Depression had flooded my soul, my thoughts were battered by negativity and I never felt safe. The aftermath, similar to a hurricane, was a life left in shambles, just wishing for normalcy.

Recent hurricanes, earthquakes and wildfires remind us of the value of life. Suicide Prevention Awareness Month provides another reminder. Every 13 minutes that the winds and rain of Irma pelted my home state, someone in this country was ending their life too soon by suicide. With a completed suicide happening in Florida every three hours , the death toll from suicide may end up being higher than that from Irma’s destruction.

Natural disasters and the realities of suicide provide the same call to action to communities. We have a responsibility to watch out for each other and take care of each other. When the power is out, you share the food and batteries you have and a reminder to hold on. When someone’s internal light is out, you share hope and support and a reminder to stay strong.

There is much work ahead to rebuild lives, homes, and communities devastated by recent natural disasters; to support communities in reducing death by suicide; and to help individuals struggling through adversity. I am living proof that a life can be rebuilt. Whatever challenge you are facing, hold on, stay strong and don’t be afraid to ask for and accept help.




Living in Recovery

September is National Recovery Month, and during this time, we celebrate the fact that people living with mental health and substance use disorders can and do recover.  Recovery is real.  But what does it mean to recover?  It’s an important question, and there is no simple answer.  Each individual experiences recovery in unique ways.  As a person in recovery, here is what I believe we have in common.

Hope – We all need hope in our lives.  Hope is like a beacon, a light that shines in our lives and in the lives of others.  It is vital.  But there are times when we lose hope.  It’s in those times we need someone to hold the hope for us.  Hope doesn’t cost anything to give, yet it is priceless.

Self-determination – Self-determination is a fundamental value in our lives. For many of us, our choices have been limited due to the impact of our mental health or substance use disorders.  Some of us live with both.  There are times when conditions are imposed on us that are not consistent with our own goals and aspirations.  Self-determination is so important, because without it, we can feel hopeless and without control in our lives.  Decision support tools and opportunities to strengthen self-efficacy empower us to choose self-determined roles in communities of our choice.

Connectedness – We are interdependent.  Connections to others and meaningful relationships are human needs.  Without connections, we can feel isolated. Developing circles of support and being included strengthens our recovery.  We are part of our communities and cultures, not separate from them.

Health  – Many of us live with chronic health conditions.  In fact, the average life expectancy for a person living with a serious mental health condition is twenty five years shorter than the general population. Finding good health care professionals who support improvements in our health and conditions, beyond just symptom and illness management, helps us realize improved personal health outcomes.   We develop healthy living habits.  Good nutrition, exercise, restful sleep; we are intentional in our approach to live well.

Peer Support – To me, peer support is the bedrock for recovery.  When someone shares experiences we can relate to, experiences we may have in common, we discover we are not alone.  We discover others have made it through similar difficult times and overcome similar challenges.  We are encouraged.  We gain confidence.  We rediscover hope.  And we pay it forward.

As I think about this year’s National Recovery Month, I know from my own experience that recovery is not a straight path.  I know there may be setbacks and hurdles to overcome.  But I am absolutely convinced that recovery is real.  It happens when we have hope in our lives, when we have choices, and when we are connected to each other and our communities.   It happens when our health care needs are met and we work to become healthier.  For so many of us, peer support represents the beginning of our journey.  Let us celebrate each person’s pathway, honor each person’s journey, and welcome those who walk alongside us.




Helping Members Change Their Lives (And Our Own Lives, Too)

As a former Medicaid recipient who relied on treatment from community behavioral health centers, I know first-hand the transformative power a peer specialist can have a in a person’s life.

In my late teens and early twenties, I bounced from state hospitals to crisis stabilization units desperate to end my life. I had no hope that the goals I had envisioned prior to these struggles could be realized. When I started working with a peer specialist, I began to view myself and my future in a more positive light. My peer specialist expected me to recover and connected me to the resources and supports that empowered my recovery process. The painful experiences of my past paired with skills and knowledge of the healthcare system have provided me a platform for me to help others while creating lasting change. The opportunity to do such meaningful, passion-filled work for more than 40 hours of my week keeps me inspired and grateful to be a part of helping our members’ lead healthy and vibrant lives.

The work we do through Magellan Complete Care of Florida’s peer navigation program is literally changing the trajectory of peoples’ lives. Peer support offers hope to the hopeless. It helps our members see new possibility in a life that feels limited. I often tell the team that they have the hardest job in healthcare, as the members we support have multiple chronic health conditions, may live in poverty, and have challenges getting their basic needs met, such as food and housing. The team is 100 percent field-based and every day they show up and not only listen and support our members, but are willing to share pieces of their own story to inspire hope and change. The results have been nothing less than extraordinary. We’ve seen decreased inpatient days and a reduction in healthcare spending, but more importantly, members are re-engaging in their community and reaching goals related to their health and wellness. We bear witness to life change as members discover their power and break through barriers to accomplish what had once been buried dreams.

These potentially life changing interactions happen every single day with our staff and the members they touch. There are a lot of examples of member success stories, but here are two that underscore the power of peer support:

  • Kevin’s low self-esteem led him to live a very unhappy and socially reclusive life. Driven by fear of the world and feelings of being uncared for, Kevin attempted suicide. Thanks to the support provided by Magellan Complete Care through a peer specialist that visits him weekly, Kevin now lives a positive and socially involved life. He enjoys spending time with his new friends and communicating with his peer specialist.
  • Joe struggled with debilitating challenges such as depression, suicidal attempts, chronic homelessness, wheelchair-boundedness, severe eye health problems, frequent hospital admissions, inconsistent medication usage and medical follow-ups. Magellan Complete Care worked with Joe to move toward the road to recovery through a complex case management program with a care coordination team and a peer support specialist. The team linked Joe to medical and psychiatric providers, a mental health agency and helped him obtain an independent living facility, food stamps, a new wheelchair, phone, eye glasses, and transportation assistance to attend appointments and follow-ups. Magellan Complete Care also assisted Joe in obtaining a referral for a prosthetic leg. Magellan then linked Joe with a job agency to support his goal of acquiring a job.

The members’ feelings of hope, pride, and self-confidence are contagious and we share these feelings when we reflect on the difference the health plan and staff makes in members lives. Our members are changing their own lives, but ours are changing along with them.




Person-first language: It’s time to bring healthcare into the 21st century

What’s in a word? Much more than many of us realize.

In the context of behavioral health, substance use and even physical health challenges, using terms and phrases that group people by diagnosis, disability, disease and other characteristics perpetuates stigma, discrimination and exclusion. Yet this type of language has been part of the healthcare lexicon for decades. Outdated terms such as “addict,” “crazy” or “diabetic” are just a few common examples.

We live in a time when individuals are at the center of the healthcare field. As healthcare consumers, individuals are empowered to make their own health choices. As healthcare professionals and activists, we need to mirror this empowerment, and seize the opportunity to pivot how we portray what we do. We need to move away from archaic language that contradicts all of the positive changes we help individuals make in their lives on a daily basis.

This is where “person-first language” can make a big difference.

What is person-first language?

Person-first language means seeing people as “people first,” and not as their disease, illness or disability, or as part of a homogeneous group. It portrays individuals living with behavioral health, substance use or physical health challenges beyond a lens of illness, diagnosis and hopelessness. It helps address issues relating to illness-identity and self-stigma, keeping in mind that we are all unique individuals, with unique lived experiences.

At Magellan Health, our use of person-first language stems from our work in behavioral health, but it applies to everything we do with equal emphasis. It shows our commitment to being culturally and linguistically appropriate in all of our communications. It models our principles of recovery and resiliency, and contributes to evolving and improving our organizational culture.

On a personal level, as an individual in recovery myself I can tell you how important person-first language became to me as I discovered the often unintended consequences of using language that robs one of their individuality. We all deserve respect and appreciation for our unique qualities.

How is person-first language used?

Using person-first language is an intentional practice. Here are some examples of old language that is commonly used in comparison to newer, person-centered language that can be applied by anyone:

  • From “chronic disease management” to “improving health outcomes for people living with chronic health conditions.”
  • From “illness self-management” to “improving health education, support and community inclusion to promote individual wellness and self-direction.”
  • From “crazy, nuts, lunatic” to “someone who may benefit from services and supports.”
  • From “individuals suffering with a mental illness” to “individuals with a mental illness.”

For practice, try to recognize when others use the outdated or inappropriate terms and phrases above. And ask yourself how often you use them. Then, make the conscious choice to omit them from your vocabulary and replace them with new terms. While changing an old habit can be a challenge, consciously developing a new one is an easier path to meaningful change.

Remember, we all have choices about the words we speak and write. Those choices can either affirm the distinctive individuals that we are — or diminish us with labels. The words we use can fill us with hope, or burden us with despair. So let’s choose hope.

Changing the way you speak and write is a gradual process. But by putting the person first when you do, you can play a role in bringing our healthcare language into the 21st century.
